Page OverviewIf your driver's license or identification card has been lost, stolen, or destroyed, you'll need to apply for a duplicate at your local Office of Motor Vehicles (OMV) location.
You'll need to take one primary and one secondary form of identification, or three secondary forms of identification. The identification documents are the same as those required when you applied for your license for the first time.
Take your marriage license, divorce decree, or other court document if your name has changed since your last license was issued.
If you're under the age of 18, you must take a certified copy of your birth certificate as well as a secondary form of identification. You must also have a parent or legal guardian accompany you to the OMV office.
Top Fees
The fee for replacing a lost or destroyed driver's license or identification card is $16. This must be paid in cash, though fees may vary depending on the OMV office.
If your driver's license or identification card was stolen and you reported it to a law enforcement officer who filed a report, the replacement is free.
Top Out-of-State Residents
As long as you're a permanent resident of Louisiana, you can still replace a lost, stolen, or destroyed driver's license or identification card if you're temporarily outside the state or country. For example, military personnel or residents who are attending school in another state or abroad can use this option.

If your driver's license or identification card is within 90 days of the expiration date, it will be renewed rather than duplicated. Please refer to our Renewing Your License section for detailed information about the renewal process.
