Fortunately, the Nebraska Department of Motor Vehicles has streamlined the process of obtaining a duplicate license.
If you're a current Nebraska resident, you can request a new license by presenting proof of identification at the office of your nearest county official. You'll need one form of identification that shows your full legal name and birth date, and two forms that prove your principal address. Only original documents will be accepted; no faxes or photocopies are allowed. Duplicate licenses cost $13.50.
Your license will be mailed to you after about one week.

College students and other Nebraska residents who are temporarily living outside the state will need to complete an Application for Revision form to receive a duplicate license. Mail the completed form with your notarized signature and proof of identification to the following address:

If you're in another country when you lose your license, you need to call your DMV exam station. The DMV will take your information, pull up your driving record, and send you the appropriate forms.
The DMV will also provide you with specific instructions on how to complete and submit the forms; however, know that you'll still be required to send a notarized copy of some form of photo identification.

After you've reported your missing license, you may also wish to place a "fraud alert" on your credit report to stop additional accounts from being opened without your permission
