When your name changes, you must notify the state of New Jersey within two weeks of the change. You'll need to visit your local Motor Vehicle Commission (MVC) Agency with the appropriate documents showing the name change.

Appropriate documents could include a marriage certificate, divorce decree, adoption or naturalization papers, or other court order, along with your current, valid driver's license.

Top Changing Your Title and Registration
To change the title and registration, you'll also need to bring along your current title and registration certificate.
When you arrive at the MVC Agency, examiners will give you the appropriate forms to complete; the change forms are not available online. Changes to all documents can made at one time.

Top Replacement Document Costs
The cost for a replacement driver's license is $11 and the cost for duplicate registration papers is $5. The cost of a duplicate title―a hard copy―is $25.
