Replacing your Oregon driver's license if it is lost, destroyed, or stolen means a trip to a regular or full-service Oregon Department of Motor Vehicles (DMV) office.

There, you must do the following, provided your driving privileges are not suspended or revoked in Oregon or other state:

• Provide proof of identity

• Provide proof of residence

• Pay the replacement fee of $25.50

If the DMV does not have a photograph of you on file, the proof of identity must include a birth certificate or identification containing a photo, such as U.S. passport, military ID, immigration document, out-of-state license, or ID card.

Top Out-of-State Travelers

If you are traveling outside the state or the country when you lose your license, you may apply by mail for a "valid without photo" driver license to use until you return. Call (503) 945-5400 to request a packet, then send back your a completed Valid Without Photo Driver License Application (noting your one-time mailing address in the appropriate box) along with a $25.50 fee to:

• Driver and Motor Vehicle Services

• Driver Issuance Unit

• 1905 Lana Ave. NE

• Salem, OR 97314

The DMV will send you a replacement in the mail.

Within 30 days of your return to Oregon, you must go in person to a DMV office to swap your "valid without photo" license for a proper photo license and pay another $25.50.

Top Reasons to Replace a License or Permit

The Oregon DMV issues replacements of non-commercial Class C driver licenses or instruction permits in the following cases:

• The license or permit is stolen, lost, or destroyed

• The license or permit holder's address has changed and a new card is desired

• Name change

• Desire to add, delete, or change descriptions, notations, photo, or signature

• Replacement is an exchange of a "valid without photo" license for one that includes a photo

• Former Oregon license holder returning from another state and there is at least one year and one month left on previous Oregon license

• Replacement of a license issued with an error

• Obtaining a license after reinstatement following suspension

• Replacement of a license confiscated by police or the courts

For your Oregon license or permit that has an illegible or mutilated address sticker and your address has not changed, a replacement sticker will be provided for free by the DMV.

Call (503) 945-5000 or fill out the general DMV Driver-related Question Form with a request in Step 4. Do not complete another Change of Address Notice if you are only requesting a replacement address sticker.
