Replacing your Oregon driver's license if it is lost, destroyed, or stolen means a trip to a regular or full-service Oregon Department of Motor Vehicles (DMV) office.
There, you must do the following, provided your driving privileges are not suspended or revoked in Oregon or other state:
- Provide proof of identity
- Provide proof of residence
- Pay the replacement fee of $25.50

If the DMV does not have a photograph of you on file, the proof of identity must include a birth certificate or identification containing a photo, such as U.S. passport, military ID, immigration document, out-of-state license, or ID card.

If you are traveling outside the state or the country when you lose your license, you may apply by mail for a "valid without photo" driver license to use until you return. Call (503) 945-5400 to request a packet, then send back your a completed Valid Without Photo Driver License Application (noting your one-time mailing address in the appropriate box) along with a $25.50 fee to:

The DMV will send you a replacement in the mail.
Within 30 days of your return to Oregon, you must go in person to a DMV office to swap your "valid without photo" license for a proper photo license and pay another $25.50.
Top Reasons to Replace a License or Permit
The Oregon DMV issues replacements of non-commercial Class C driver licenses or instruction permits in the following cases:
- The license or permit is stolen, lost, or destroyed

For your Oregon license or permit that has an illegible or mutilated address sticker and your address has not changed, a replacement sticker will be provided for free by the DMV.
